Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subgrantee will be responsible for administering the Volunteer Transportation Services program <br /> in a manner satisfactory to the City and consistent with any standards required as a condition of <br /> providing these funds. Such program will include the following activities eligible under the Community <br /> Development Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> [Complete description of activity to be undertaken including what projects, products or services are to be performed, <br /> where they are to be provided, for whom they are to be provided, how they are to be provided] <br /> Volunteer Transportation services provides elders and disabled adults with the transportation they <br /> need to be able to get to medical appointments, the grocery store, and to other essential errands. <br /> Clients call out Transportation Coordinator at least one week prior to their requested trip date and <br /> provide times, destination, and reason for the trip. Volunteers are then assigned to clients based on <br /> geographic location of the driver and client, as well as the trip destination. <br /> Our volunteer drivers are generous, well-trained individuals who often build a close, ongoing <br /> relationship with the clients they serve. Some of our clients have disabilities including mental health <br /> issues that prevent them from accessing public transportation. Having a reliable, consistent volunteer <br /> driver provides them support while completing daily tasks. <br /> There are no other agencies that provide volunteer transportation services to as broad a client base <br /> and geographical area as CCS. The other volunteer transportation services in Snohomish County <br /> are devoted to cancer patients and provide them with delivery to cancer treatments exclusively. <br /> Other transportation services serving the same client base have specific requirements regarding <br /> service area and accessibility, often disqualifying many of the clients we serve. They also tend to <br /> offer a window of time for service rather than a specific pick up time, which is something that is <br /> nearly impossible for many of our clients to work with. Many of our clients are unable to wait outside, <br /> or on their feet, or without access to a restroom for long periods of time. <br /> A-1 <br />
