D. Levels of Accomplishment - Goals and Performance Measures In addition to the normal administrative services required as part of this Agreement, the Subgrantee agrees to provide the following levels of program services: Activity Units of Service* per Month Total Units/Year Miles per month 1000 10, 000 Total People or Households to be served Total unduplicated Everett Residents or Households to be served 40 30 *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, persons or households assisted, or meals served, here. If this is a construction project, please explain here how the improvements will be used and how they benefit the community, what the target population is for the project, etc. The Subgrantee agrees to track outcome performance measures as required by the U.S. Department of Housing and Urban Development and the City. The Subgrantee will report the outcomes to the City on at least a quarterly basis. Information and forms will be provided to the Subgrantee by the City for reporting this information. E. Staffing [Add Provide a list of staff and time commitments to be allocated to the activity specified in I.A. above] The program is staffed with 1 part-time coordinator who focuses on Transportation Services exclusively. Funding in this proposal would supplement funding for the coordinator, not to exceed 30% of the coordinators' work hours. II. Time of Performance Services of the Subgrantee shall start on the 1st day of August, 2016 and end on the 31st day of May, 2017. The term of this Agreement and the provisions herein shall be extended to cover any additional time period during which the Subgrantee remains in control of CDBG funds or other assets, including program income. The City may, at its discretion, extend the term of this Agreement to allow for the expenditure of unexpended funds. A-3
