DESCRIPTION/JUSTIFICATION OF AMENDMENT, MODIFICATION, OR CHANGE ORDER: Under the authority of Presidential Major Disaster Declaration FEMA 4056-DR-WA, the Department through its Public Assistance Program, is reimbursing the Applicant for those eligible costs and activities necessary for the repair and restoration of public facilities damaged during this disaster. Due to circumstances beyond the applicant's control,the repair and restoration to all of the damaged public facilities have not been completed. An extension of the period of performance is allowable under grant provisions. 11. AMENDMENT TERMS AND CONDITIONS: 1. Change the grant expiration date from March 5, 2016 to March 5, 2018. No other changes are required. All other terms and conditions of the original grant and any previous amendments thereto remain in full force and effect.
