Chi•is Buins, Project Superintendent �vith <br />McBride Construction, met �vith DEI on site and provided access to the bui(ding in addition to information about <br />the construction. Micah Olson, with Advanced Foundation Supports, also met with DEI �n site and provided <br />information on the pile installation. DEI's scope for this project included reviewing the helica] pile ciema��d, <br />visiting the site to observe existing conditions, ai�d providing direction when requested by the project team during <br />COIISlI'UCi1011. <br />DEI's observed the followin� elemen#s on the March 18 visit: <br />IIelical Piles <br />• (50) piles at the continuous spread footings <br />• Piles spaced at 4'-5' on center at the perimeter spread footings <br />• Piles spaced at 30" on center at ttle interior spread footings <br />• (3) piles at an isolated spread footing adjacent to the utility room <br />The helical piles observed appear to be structurally complete and in confor�nance with the observations in Nelson <br />Geoteclmical Associates' report dated March L8, 2014 ancl t}�e recommendations in Structiu•al Design Associates' <br />revised plan dated March 13, 2013. <br />Pin Piles <br />• (4) piles at each of the (4) isolated spread footings at �vest end <br />The pin piles observed appear to be structw-ally complete and in conformance with the observations in Nelson <br />Geotechnical Associates' report dated iv1a�•cl� 18, 2014 and the recommendations Dibble Engineet�s' Structurai <br />Review Swl�mary dated Februaty 24, 2014. <br />1029 �L��rr�ket Str�ect Page 1 of 2�� P. �25-82J'-4200 <br />Kirklafid, Wil 95033 F. =/2 �-827-6131 <br />
