ARTICLE 19 - INSURANCE BENEFITS <br /> 19.1 General. <br /> 19.1.1 The City agrees to provide insurance benefits for employees and their dependents <br /> as described in the following sections. <br /> 19.2 LEOFF I - Dependent Medical. <br /> 19.2.1. Effective July 1, 2015, the City agrees to offer medical insurance and a vision plan <br /> that is acceptable to the City for LEOFF I eligible dependents, provided the <br /> employee makes a ten percent contribution toward the cost of the medical <br /> coverage. <br /> The City will pay the monthly premium cost toward the purchase of the City's <br /> revised self-insured medical plan for employees and their dependents. <br /> If the health management organization plan is elected, the employee shall pay the <br /> premium difference between the City's self-insured plan and the health <br /> management organization plan. <br /> 19.3 LEOFF II - Employee and Dependent Medical. <br /> 19.3.1 Effective July 1, 2015, the City agrees to offer medical coverage for employees and <br /> their eligible dependents, provided the employee makes a ten percent contribution <br /> toward the cost of the medical coverage. If the employee chooses a carrier other <br /> than the City's self-insured program, the employee shall pay the premium <br /> difference. <br /> 19.4 Dental Insurance. <br /> 19.4.1 The City agrees to provide dental insurance through Washington Dental Service for <br /> all employees and their dependents. The City shall pay the premium cost of the <br /> coverage. Effective January 1, 2007 the annual maximum shall be increased to <br /> $2,000. <br /> 19.5 LEOFF II -Vision Insurance. <br /> 19.5.1 The City agrees to provide vision coverage that is acceptable to the City for all <br /> LEOFF II employees and their legal dependents. The City shall pay the premium <br /> cost of the coverage. <br /> 19.6 Disability and Life Insurance. <br /> 19.6.1 The City agrees to sponsor and administer a disability insurance program through <br /> Standard Insurance Company for all LEOFF II employees. Premiums for this <br /> coverage will be the responsibility of LEOFF II employees with the requirement that <br /> all LEOFF II employees participate. <br /> EVERETT POLICE MANAGEMENT ASSOCIATION <br /> PAGE 23 <br />
