The construction documents have been reviewed and construction <br />can begin, si�bject lo conslruction pe�mitting fi�om the local birilding o�cial. This project will not be approved until <br />tlie comments labcled as "not approved" have been resolved by providing a written response to each of tlle <br />comments. When referencii�g drawings o�• attached materials, provide sl�eet or page #, revision # and date as <br />applicable. Additional comme»ts may follow based on desig�� team response and site inspections may be req�iired to <br />verify compliance. <br />Plc�isc notc thc f��ll��wing: <br />■ Tl�e stamped "Authorization to Begin Construction" drawings will be delivered to the Facility Administrator. <br />These shall be kept available on site for inspection dL�ring construction and post occupancy survey. <br />■ Any changes/deviations (incl. change orders or addenda) froin the approved documents must be submitted to <br />the Department for review and approval. <br />■ Beginning construction prior to resolving the attached comments will constitute facility acknowledgement that <br />you are proceeding at your own risk. <br />■ Please include your CRS n�unber on all commlinications to Construction Review Services. <br />PROJECT CLOSF-OUT REQUIREMENTS You must notify the departillent when <br />construction is complete by completing the following steps: <br />✓ Verify tliat you have resolved all of the com�nents on tlzis form and have submitted any <br />revisions <br />✓ Coinplete tlle Onliile Notification of Construction Completed at: <br />http://www.doh.wa.gov/NotificationofCoinpletion.aspY <br />✓ F,mail or fax a copy of the approval from the local b�iilding department (final pennit <br />approval or certificate of occupancy); and <br />✓ Email or faX a floor plan showing tl�e scope of work. <br />✓ Einail: fslcrs�doh.wa.gov Fax: 360-236-2321, Attn: Construction Review <br />Once your construction project is complete, you may contact the DOH Office of Customer Service for help <br />with adjusting or amending your license to add this project.
