e) Provide Certificate of insurance for the duration of the contract listing City of <br /> Everett as additionally insured. If the insurance expires prior to the end of the <br /> contract, a new Certificate of Insurance must be received by the Point of Contact <br /> by the expiration date of the previous term. <br /> f) Provide an Additional Insured Endorsement on the insurance as stated in <br /> Specific Requirements 2b. <br /> 4. Provide leadership and planning for outdoor activities. Hourly rate for these <br /> activities will be at $13.00/hr. Hours per trip are agreed to in advance between the <br /> service provider and point of contact. <br /> a) Includes: hikes, tours, skiing, snowshoeing, biking, trips and tours and outdoor <br /> school. <br /> a) Develop trips: planning dates, times, routes, and best activities. <br /> b) Provide equipment lists and agendas as needed. <br /> c) Lead pre-trip planning session with participants. <br /> d) Provide trip evaluation and report after each trip. <br /> e) Develop curriculum to instruct outdoor activities. <br /> f) Monitor and evaluate students' progress. <br /> g) Maintain and submit course evaluations. <br /> h) Maintain course attendance sheets. <br /> i) Monitors students and activity for safety. <br /> j) Compile annual trip summary. <br /> k) Write promotional information for activities <br /> I) Drive 15 passenger van and complete all required training <br /> 5. Provide Leadership and planning for day camps as day camp director. Hourly <br /> rate for these activities will be $15.00/hr. <br /> Includes: Science day camps, and other day camps as assigned. <br /> a) Develop program curriculum <br /> b) Act as day camp director. These duties include: <br /> 1. Assist with hiring and training camp staff <br /> 2. Provide program evaluations and report <br /> 3. Develop camp procedures <br /> 4. Provide direct program leadership <br /> 5. Supervise and evaluation of day camp staff <br /> 6. Coordinate camp transportation and program reservations <br /> c) Monitor and evaluate students' progress. <br /> d) Maintain and submit course evaluations. <br /> e) Maintain course attendance sheets. <br /> f) Monitors students and activity for safety. <br /> g) Write promotional information for activities <br /> h) Other responsibilities as requested by point of Contact. Additional <br /> responsibilities will be provided in writing to service provider and will include <br /> max hours allotted for tasks and deadlines prior to the start of any additional <br /> billable work. <br /> 6. Dates and times (see attached outdoor trips and classes) <br /> Hourly rate for each trip as an activity leader as referenced in #2 will be based on the <br /> following.
