A. Has been convicted within the last ten years of any felony that directly <br /> relates to the teaching position. By way of example only, such felonies <br /> might include: crimes against vulnerable persons, such as children, the <br /> elderly, or the disabled; crimes of dishonesty; or crimes using, or <br /> threatening, violence, including, but not limited to, the use, display or <br /> threat of a weapon. <br /> B. Has behaved in such a way that the Service Provider reasonably <br /> concludes that the proposed teacher does not possess the skill, care and <br /> judgment necessary to be a recreation teacher with due regard for the <br /> students, facility, or other persons who may be in or near the class. <br /> 25. CERTIFICATION AND DOCUMENTATION required as follows: <br /> a) EYes No Current First Aid/CPR card required for a staff on <br /> site during the instruction of these classes. <br /> b) Provide Certificate of insurance for the duration of the contract listing <br /> City of Everett as additionally insured (#11.A2) If the insurance <br /> expires prior to the end of the contract, a new Certificate of Insurance <br /> must be received by the Point of Contact by the expiration date of the <br /> previous term. <br /> c) Provide an Additional Insured Endorsement on the insurance as <br /> stated in contract (#11D). <br /> d) Provide a UBI number (#16). <br /> e) Return two signed and completed original contracts <br /> Specific Requirements for GYMaqine Gymnastics <br /> 1. Provide the following for all classes taught <br /> a) This Professional Services Agreement is effective when the provider receives a <br /> written "Notice To Proceed" with a specific date from the provider's Point of <br /> Contact. <br /> b) Plan and implement Preschool and Youth level Gymnastics classes <br /> c) Develop curriculum for classes with a focus on core gymnastics <br /> d) Maintain attendance records and provide them to EPRD <br /> e) Assist with promotion of other park programs by providing park guides or other <br /> program flyers at GYMagine Gymnastics. <br /> f) Provide a clean, safe and welcoming facility for all classes <br /> g) Monitor and evaluate student's progress <br /> h) Maintain and submit course evaluations <br /> i) Develop other programs and classes as agreed upon with the parks contact <br /> j) Prepare class materials and handouts for each class <br /> k) Inform the parks contact if situations with customers generate a complaint with <br /> regards to program content, staff or other issues <br /> I) Renew Annual City of Everett Business License for 2017, no later than January <br /> 31, 2017 <br />
