AMENDMENT NO. 10 <br /> TO <br /> AMBULANCE TRANSPORT BILLING SERVICES AGREEMENT <br /> BETWEEN THE CITY OF EVERETT <br /> AND SYSTEMS DESIGN NORTHWEST,INC. <br /> This Amendment is dated for reference purposes January 1, 2017. It is by and <br /> between the City of Everett, a municipal corporation under the laws of the State of <br /> Washington("City") and Systems Design Northwest, Inc. ("Service Provider"). <br /> RECITALS <br /> A. The City and Service Provider are parties to the Ambulance Transport <br /> Billing Services Agreement signed by the Mayor on March 28, 2005, as amended by <br /> Amendments Nos. 1-9 (as amended by such amendments, the "Agreement"). The <br /> Agreement was originally awarded in 2005 pursuant to Request for Proposals 2004-737. <br /> B. The purpose of this Amendment is to extend the expiration date of the <br /> Agreement. <br /> AGREEMENT <br /> The City and Service Provider agree as follows: <br /> 1. The Agreement is amended so that the Agreement remains in effect until <br /> December 31, 2017. <br /> 2. Regardless of the date(s) on which this Amendment is signed by the parties, the <br /> parties agree that the Agreement shall be deemed to have been continuously in <br /> effect since April 1, 2005. <br /> 3.
