Applications will then be reviewed against the application <br /> checklist. Upon review by the Washington State Library, if the answer to any one <br /> of the questions in this checklist is "no", the application may be rejected without <br /> further consideration. <br /> CRITERIA / QUESTION YES NO <br /> An original application and one copy were submitted.
