Beech St. <br />Permits: E0404-125 (Wire addition — last inspection 6-14-04) <br />E0406-066 (Replace 100 amp panel — no inspections on record) <br />(You must reference both permit number and address when calling or writing) <br />According to our records there has been no request for inspection in the last 180 days. Please sec <br />the enclosed inspection activity reports. The International Building Code and the International <br />Residential Code state that all permits expire if the work authorized by the pcmmits is suspended <br />or abandoned for a period of 180 days after the work is commenced. Work under these permits <br />will no longer be authorized unless you: <br />l) Demonstrate continuing progress by requesting inspection of work that has occurred since <br />your last inspection, or <br />2) Request final inspections of the work that is completed, or <br />3) Submit a written regi:est for a 180 day extension, detailing justifiable cause for extension. <br />Please respond within 10 days. After that time your permit file will be sent to our Records <br />Department for microfilming without fina! approval. These records are available to prospective <br />buyers and lending institutions. If you wish to resume work in the future, a new permit must be <br />obtained.
