The Health Distric[ operating pertnit application process must be completed prior to opening for <br /> business. <br /> 2. A reduced pressure backflow prevention device is required at the end of the copper water pipe serving the <br /> pop dispensmg system prior to the carbonation device. <br /> 3. A horizon[al separation of at least 16 inches or a vertical partition l6 inches in height is required between <br /> the food preparation sink and all other sinks or sources of contamination. <br /> 4. Water heaters musf be of su�ficient size to provide hot water to dishwasher and/or scullery sinks and at the <br /> same time provide hot water to all handwash sinks. <br /> 5. All food service equipment must be listed by the National Sanitation Foundation (NS� for its intended <br /> use. <br /> 6. Storage refrigerarors must be listed for the storage of potentially hazardous foods. <br /> 7. Plumbing must meet state and la_aI codes. <br /> �. The ventilation system shall be i:istalled and operated to meet applicable building, mechanical, and fire <br /> codes. <br /> A preoperational inspection is required prior to opening for business. At the time of inspec[ion the <br /> construction of the food service establishment must be complete and all equipment must be in place. <br /> Incomplete construction may result in a 562.00 reinspection fee. Please contact the Food Program oftice at <br /> Ieast one week in advance lo schedule an appointment. This will ensure compliance with the Rules and <br /> Re�ulations of the State Board of Health for Food Service Sanitation. <br /> If there are any significant changes or additions to your layout or equipment, the Snohomish Health District <br /> must be notified. <br /> Please contact me if you have any questions.
