ENVIRONMENTAL CHECKLIST <br />Purpose o(Checklist: <br />The State Environmental Policy Act (SEPA), chaptcr 43.21C RCW, requires all governmental agencia to consider <br />the ervironmental impacts of a proposal before making decisions. An environmental impact statemenc (EIS) must be <br />prepared for all proposals with proba6le significant adversc impacu on the quality of the environment. The pu:nose o( <br />this checklist is to provide in(ormation to help you and the agency identi(y impacts (rom your proposal (and to re�uce <br />or avoid impacu from the proposal, i( it can be done) and to help tlie agency decide whether an EIS is required. <br />Instructions ior Applicanu: <br />This environmental checklist asks you to dacribe some basic information about your proposal. Governmental agen- <br />ciu use this checklist to determine whether the environmental impacu of your proposal are significant, requiring <br />preparation of an EIS. Answer the questions briell�, with the most precisa informacion known, or give the bst de- <br />scription yau can. <br />You must answrr each ques�ion accurately and carefully, to the bat of your knowlcdge. In most cases, you should <br />be able to answer the questions trom your own observations or project plans wi[hout the need to hire experts. If you <br />rra.11y do not know the answer, or if a question does noc apply to your proposal, write "do not know' or 'dou not ap- <br />ply'. Complete answers to the quations now may avoid unnecessary delays later. <br />Some ques[ion� ask about governmental rcgulations, such as zoning, shoreline, and landmark daignations. Answer <br />thae quations if you can. If you have problems, the governmental agenciu can auist you. <br />The checklist quations apply tu ali parts of your proposal, even i( you plan to do them over a period of time:or on <br />di(ferent parcels of land. Attach any additional information that will hclo ducribe your proposal or its environment�l <br />effecu.
