INSPECTION <br />3200 CEDAR STREET <br />EVERETT, WA 98201 <br />Attention: Tim Alaniz <br />Subject: Power duct bank MHE 140 <br />�40-04 Building Permit #4902 <br />_ ---ill <br />Gentlemen: <br />July 21, 1992 <br />Job #1530 <br />It is our understanding that the City of Everett Electrical Inspector was not <br />notified of the duct bank pour and as a result was not able to witness the <br />installation. <br />Please be informed that the power duct bank on the 40-04 building project was <br />installed per plans and specification. The installation was inspected by an <br />independent testing agency, Professional Services Industries, Inc. <br />Between 06/17/92 and 06/19/92, Evergreen Utilities mandrel!ed the duct bank <br />and cleared two obstructions.
