Page 2 <br /> 1.0 PURP08E <br /> The purpose of this field evaluation is to provide assurance <br /> • that c �stom or non-certified equipment meets the <br /> requirements of the appropriate codee, safety orders and <br /> industry standards. This evaluation is normally required by <br /> , local buildinq inspection agencies when equipment has not <br /> been certified by Underwriter's Laboratory (UL) or oth^r <br /> recognized testinq agencies. This evaluation addressc•� only <br /> the electrical system on the �ubject equipment. <br /> 2.0 BUNM7IRY <br /> � This contract was initiated at the request of Mr. Bob <br /> Walter, Vice President of Industrial Electric Manufacturing <br /> (IEM) . Preliminary equipment evaluation was performed at <br /> the IEM facility in Fremont, California on December 20, 1993 <br /> + by Electro-Test Field Engineer Wilson Benosa. Due to the <br /> incomp].ete construction of these units at the time of our <br /> initial inspection, additional field inspections were <br /> � required on December 24 and December 27, 1993 . Final <br /> inspection and verification of equipment modifications was <br /> performed by Elactro-Test Engineer Bob Ruxlow on December <br /> , 27, 1993 , at the eoeing facility in Everett, Washington. <br /> Results of manufacturer's production testing was received on <br /> :1Yri1 5, 1993. <br /> � Several equipment discrepancic•s wer� observed as described <br /> ! � in Section 7.0 of this report. All discrepancies have been <br /> corrected and verified and we now consider this equipment to <br /> „ be in compliance with the electrical codes and standards <br /> � listed in Section 4.0. ETZ field evaluation labels have <br /> been installed on this equipment and a copy of this final <br /> report issued to the City of Everett. Note that final <br /> ` approval of. this equipment is by the City of Everett. <br /> , 3.0 (iENER]►L CONDITIONB OF ACCEPTANCE <br /> io 3 . 1 In the event of modifications resulting in a change in <br /> the materials, manufacturing methods, loading or <br /> environment that would affect the use of the accepted <br /> i � product under the provisions of the noted electrical <br /> stan3ards, this acceptance will be considered <br /> automatically canceled.
