Page 2 1.0 PORPOBE The purpose of this field evaluation is to provide assurance that custom or non-certified equipment meete the requirements of the appropriate codes, safety orders and industry standards. This evaluation is normally required 'py local building inspection aqenciea when equipment has not been certified by Underwriter�s Laboratory (UL) or other � recognized testing aqencies. Thie evaluation addresses only the electrical syatem on the subject equipment. 2.0 80MMARYs This project was initiated at the request of Mr. Jim Norton of Northcoast Electric. Inspection of this equipment was performed on May 3, and May 12, 1993, by Electro-Test Field Engineers Bob Ruxlow and John Schubert. Some discrepancies were observed as deacribed in Section 7 .0. All disc.repancies listed in Section 7.0 have been satisfactorilv corrected and verified and we now consider this equipme;it to be in compliance with electrical codes and standards listed in Section 4 .0, pursuant to the conditione of acceptatice described in Section 3.0. An ETI equipment label has been aff:.xed to each of the four (4) switchea evaluated.
