Page 2 <br /> 1.0 PORPOBE <br /> The purpose of this field evaluation is to provide assurance <br /> that custom or non-certified equipment meete the <br /> requirements of the appropriate codes, safety orders and <br /> industry standards. This evaluation is normally required 'py <br /> local building inspection aqenciea when equipment has not <br /> been certified by Underwriter�s Laboratory (UL) or other <br /> � recognized testing aqencies. Thie evaluation addresses only <br /> the electrical syatem on the subject equipment. <br /> 2.0 80MMARYs <br /> This project was initiated at the request of Mr. Jim Norton <br /> of Northcoast Electric. Inspection of this equipment was <br /> performed on May 3, and May 12, 1993, by Electro-Test Field <br /> Engineers Bob Ruxlow and John Schubert. <br /> Some discrepancies were observed as deacribed in Section <br /> 7 .0. All disc.repancies listed in Section 7.0 have been <br /> satisfactorilv corrected and verified and we now consider <br /> this equipme;it to be in compliance with electrical codes and <br /> standards listed in Section 4 .0, pursuant to the conditione <br /> of acceptatice described in Section 3.0. An ETI equipment <br /> label has been aff:.xed to each of the four (4) switchea <br /> evaluated. Note that final approval is under the <br /> jurisdiction of the City of Everett. <br /> 3.0 (iF.NERl1L CONDZTIONB OF 11CCEBTJ1NCLt <br /> 3. 1 in the event of modifications resulting in a chanqe in <br /> the materials, inanu.facturinq methods, loadinq or <br /> environment that would affect the use of the accepted <br /> product under the provisiones of the noted electrical <br /> standards, this acceptance will be considered <br /> automaticall}� cancelled. The applicant will be <br /> required to request re-examination of this product to <br /> determine acceptabflity. <br /> 3. 2 ETI's acceptance of the equipment referenced in Section <br /> 5.0 doea not as�tume or discharge the responsibility of <br /> the equipment manufacturer, installer or other relevant <br /> parties. <br /> 3. 3 Equipment evaluation is based upon adherence to sound <br /> enqineerinq practices, and upon compliance with the <br /> specific sections quoted from thP electrical standards <br /> referenced in Section 4 . 0 of this report. <br />
