Project: <br /> Change Order no. 0 <br /> Change Order Effective Date: <br /> Exhibit A—Description of Changed Work <br /> A. The item being added by this change order is listed below. One new bid item is being added to the <br /> contract by this change order. <br /> Item#14—Additions and Modifications to the Contract. Create a new bid item#14 to pay for the <br /> installation of two each 4-inch diameter blow-offs onto existing cross-town pipeline,installation of blow <br /> off manholes,connecting pipe and valve between blow offs and blow off manholes,and associate <br /> restoration work along the existing cross-town pipeline on 25th Street. <br /> B. Justification of the item being added by this change order <br /> Item#14—Additions and Modifications to the Contract. Sections of the existing cross-town pipeline <br /> contained water in it that could not be drained,as there were no existing blow offs,or in the case of the <br /> existing blow off at North Junior middle school,the school district had constructed a portable classroom <br /> over it making it inaccessible. The City directed Tri-State Construction Inc.to install two each blow offs in <br /> low spots along the pipe alignment. This will allow the future contractor that will be contracted with by the <br /> City to install the new cementitious coating,the ability to immediately drain and begin work on this <br /> existing pipe. <br /> C.
