PURPOSE FOR CONTRACT AMENDMENT: BUDGET MODIFICATION The amount of the contract is modified as follows: The total funds to be reimbursed to the CONTRACTOR for costs incurred during the contract period shall be a sum not to exceed$64,500 which represents an addition of $53,000 in state funds over the original$11,500 award. The Budget will be modified to reflect the line item changes as shown in Attachment B: Revised Budget. The current Workplan will not be changed and will continue to be in effect. A copy of this contract amendment shall be attached to and made a part of the original contract between the DEPARTMENT and the CONTRACTOR. Any reference in the original contract to the"CONTRACTOR"shall mean the"CONTRACT AS AMENDED." All other terms and conditions of the original contract shall remain in full force and effect.
