With the addition of the following, the plans arc <br /> approved. <br /> 1. Extra wall protection is required on walls behind sinks and food preparation areas. A 16-inch high <br /> backsplash of plastic laminate, fiberglass reinforced plastic or equal �s acceptable. <br /> 2. The floor must be surfaced with a durable, nonabsorbent, easi!y cleanable material. <br /> 3. The ceiling above the food preparation areas must be nonabsorbent, smooth, and easily cleanable. <br /> 4. All lioht fixtures in food preparation and storage areas must be provided with covers or shatterproof <br /> bulbs. <br /> 5. Your proposal to use the Herfy's restaurant, currently not in operation, as your commissary is <br /> acceptable with the following conditions. Should at any time in the future the ownership of the <br /> Herfy's chanoe and or the buildin� use changes, then Hot Rod Espresso must immediately cease all <br /> operations until a suitable commissary is found and proper documentation is submitted te ine Henith <br /> Dis[rict. <br /> A preoperational inspection is required �rior to opening for business. At the time of insoection the <br /> construction of the food service establishment must be complete and all equipment must be in place. <br /> Incomplete construction may result in a $130.00 reinspection fee. Please contact the Food Yrogram <br /> office a minimum of one week in advance to schedule an appointment. This will ensure con:�liance <br /> wi[h the Rules and Reeulations of the State Board of Health for Food Service Sani[ation. <br /> If there are ar.y significant changes or additions to your layout or equipment, the Snohomish Health <br /> District must be notified. <br /> Please contact me if you have a y questions.
