The following information is needed prior to further plan review. <br /> 1. The handwash sink (item #7) previously located next to the three-compartment sink (item #4) has <br /> been removed and replaced with a mop sink (item #11). A handwash sink is required in the three- <br /> compartment sink area. Restrooms are now shown on the floor plan, however it is unclear how customers will access the <br /> restrooms. The access way to the restrooms must be si�own on the <br /> Noor plan. <br /> 4. As noted on the plan disapproval letter of October 24, 2011, gyros are indicated on the proposed <br /> menu and HACCP. What will be done with left over gyro cones? <br /> 5. Submit a revised floor pian, drawn to scale, showing location of all equipment, plumbing fixtures <br /> and the lihe that includes the required additional sinks and information. The scale of the drawing <br /> must be no smalier than '/< inch equals 1 foot. <br /> Please note that prior to operating permit issuance and approval to open the new facility, after the <br /> Health District plan review process is completed and construction is finished, the Health District permit <br /> application process must be compieted and a preoperational inspection must be conducted. <br /> Please contact me if you have any questions.
