W�th the addit�on of the following, the plans are <br />approved. <br />1. Menu is limited to prepackaged, pre-prepared (ie., reheat and serve) food items. <br />2. Additional requirements for sinks and equipment will be needed if on site food preparation <br />and/or cooking is done at this establishment. <br />If there are any significant changes or additioas to your layout or equipment, the Snohomish <br />Health District must be notified. <br />A preoperational inspecdon is required prior to opening for business. At [he time of inspection the <br />construcGon of the food service establishment must be complete and all equipment must be in <br />place. Incomplete construction may result in a$55.00 reinspecfion fee. Please contact the Food <br />Program office about one week in advance to schedule an appointment.
