Remodel of Kadiak Ron's Pub, 7601 Evergreen Way #Kl 1201, Everett <br />De;ir Mr. McWain: <br />Your plans have been reviewed with the Rules and Reeulations of die State Board of Health, and with the policies <br />of the Snohomish Health District. With the add�t�on ot the tollowing, the plans are approved. <br />1. The Health District operating pertnit application process must be completed prior ro opening for business. <br />2. An indirect waste is required for the food preparation sink, mechanical dishwasher, ice machine, running <br />water dipper we1L and equipment in which food is placed. <br />3. A horizontal separation of at least 16 inches or a vertical partition 16 inches in height is required benveen the <br />food preparation sink and all other sinks. <br />d. Water heaters must be of suf6cient size to provide hot water to dishwasher and/or scullery sinks and at the <br />sam� time provide hot water ro all handwash sinks. <br />�. All food sen�ice equipmen[ must be listed by the National Sanitation Foundaiion (NSF) for its intended use. <br />6. Storage refrigerarors must be listed for the s[orage of potentially hazardous fcods. <br />7. Extra wall orotection is required on walls behind sinks and food preparation tables. A 16 inch high <br />backsplash of plastic laminate, riberglass reinforced plastic ar equal is acceptable. Floor to ceiling protection <br />is required on wall behind dishwashers. Plastic coated hardboard is not acceptable. <br />8. 'Che floor must be surfaced with a durable, nonabsorbent, easily cleanable material. <br />9. The ceiling above the food preparntion areas must be nonabsorbent, smoodi, and easily cleanable. <br />10. All light fixtures in Food preparation and srorage areas must be provided with covers or sha[ter proof bulbs. <br />I I. Plumbing must meet state and local codes. <br />I^_. The ventilation system shall be installed and cperated to meet applicable building, mechanical, and fire codes. <br />A preoperational inspection is required prior to opening for business. At the time of inspection the construction <br />of die food service establishment must be complete an� all equipment must be in place. Incomplete construction <br />mav result in a�62.00 reinspection fee. Please contact the Food Program oPtice at Ieast one week in advance to <br />schcdule an appointment. This will ensure campliance with the Rules and Resulatians of the State Board of <br />HealAi for Food Service Sanitation. <br />[f there are any significant changes or additions ro your layout or equipment, the Snohomish Health District must <br />be notitied. <br />Please c�act me if you have any questions.
