Based on the plans submitted and a personai visit to the <br />site by John Sipkens on May 8, 2012, the plans are approved with the following conditions: <br />�, BBQ pig and BBQ duck are approved for cooking and service, without any cooling. <br />2. The fish department is to cook fish to order for the customer without hot holding or cooling of the <br />fish. <br />3. The fish tanks must be clean and clear visually at all times. If the fish tank water is found to be <br />cioudy, the fish tanks may be require to be removed from service. <br />The Health District operating permit application process must be completed prior to opening for <br />business. This facility will be classified as a grocery with seafood and meat departments. The seafood <br />and meat departments will be medium risk, 0-12 seats. The grocery will be a low risk food <br />establishment with 0-12 seats. <br />A preoperational inspection is required prior to operating permit issuance and approval to open <br />for business. At the time of inspection the construction of the food service establishment must be <br />complete and all equipment must be in place and in proper operating condition. Incomplete construction <br />or equipment operation will result in a$168.00 re-inspection iee. Contact the Food Program office a <br />minimum of one week in advance to schedule an appointment for the pi•eoperational inspection. <br />The preoperational inspection will ensure compliance with the Rules and Requlations of the State <br />Board of Health for Food Service Sanitation. <br />Changes or additions to the approved plans or equipment require pre-approval from the Snohomish <br />Health District prior to implementation of the changes. <br />Please contact me if you have any questions.
