Snohomish Health District. <br />THRESHOLD DETERMINATION <br />4i_rr <br />The lead agency for this proposal has determined that it does not have a probable significant adverse <br />impact on the environment. An Environmental Impact Statement is not required under RCW <br />43.21C.030(2)(c). This determination assumes compliance with State law and City ordinances related <br />to general environmental protection including but not limited to right-of-way improvement <br />requirements, drainage, etc. This decision was made after review of a completed environmental <br />checklist and other information on file with the lead agency. This information is available to the public <br />on request. This Mitigated Determination of Non -Significance is specifically conditioned on compliance <br />with the conditions attached hereto which are incorporated by reference as if fully set forth herein. <br />MITIGATION MEASURES <br />This threshold determination is issued with the implementation of the following mitigation measures. <br />1. Prior to issuance of any permits, the Applicant must pay a mitigation fee of $31,426 per the <br />City's Traffic Mitigation Ordinance No. 2425-99. <br />This Proposed MONS is issued under 197-11-355.
