With the addition of the following, the plans are approved. <br />1. The Health District operating permit application process must be completed prior to opening for <br />business. <br />Z. Prior to pemut issu�nce a copy of the menu must he submitted. This may be submitted at time of pre- <br />operational inspection. <br />A preoperational inspection is required prior to upening for business. At the time of inspecdcn the <br />cons�rucuon of the food service estabGshment must be wmplete and all eyuipment must be m place. <br />Incompl..te construction may result in a$62.00 reinspection fee. Please contact the Food Program <br />office a minimum of one week in advance to schedule an appointment.
