There is no <br />record of a previous food service establishment at this location. As such there is no preexisting <br />equipment. The manufacturer names and model numbers for all equipment noted as existing must <br />be submitted. <br />12. No dry storage area in included on the floor plan. A dry storage area is required and the location <br />must be shown on the floor plan. <br />13. No plumbing schedule (i.e. direct and indirect drain locations) was submitted. The plumbing <br />schedule must be submitted. <br />14. The deli menu indicates that chicken, beef, ham, and cheeses served. Is the storage of both raw <br />meat and ready to eat food proposed in the same case? If so, how will cross -contamination be <br />prevented? Will deli sandwiches be served? If so a revised menu must be submitted for the deli. <br />15. No hood construction details were submitted. The hood construction details must be submitted. <br />16. All food service equipment must be listed by the National Sanitation Foundation (NSF) or equivalent <br />for its intended use. The Merco/Alto hood model # SM-4-5 item #5 the Merco/Alto food warmer <br />model # SM-4-5 item #6, and the Nor -Lake refrigerator model #NIR2 were not found in the current <br />NSF or equivalent listings: This equipment must be replaced with NSF or equivalent listed <br />equipment or documentation must be submitted which demonstrates NSF equivalency. <br />Manufacturer names and model numbers for replacement equipment or equipment documentation <br />must be submitted. Originally submitted manufacturer names and model numbers should be <br />rechecked to assure the information submitted is accurate. Please note that subsidiary <br />17.
