The Health District operating permit application process must be completed prior to opening for business. <br /> 2. An indirect waste is required for the food preparation sink, mechanical dishwasher, ice machine, running <br /> water dipper well, and equipment in which food is placed. <br /> 3. M additional handwash sink is aquired at the baz. The best locaaon would be at the end of the work deck <br /> (Item 60, Sheet EQ-1) next to the lounge access point, or rolocate handsink (Item 52), currently shown at <br /> pull tab entrance to bar, to a more central location on the work deck (Item#60). <br /> 4. All food service equipment must be listed by the National Sanitation Foundation (NSF) for its ietended <br /> use. <br /> 5. Storage refrigerators must be listed for the storage of potendally hazardous foods. <br /> 6. Extra wall protection is required on walls behind sinks and food preparation tables. A 16 inch high <br /> backsplash of plastic laminate, fiberglass roinfocoed plastic or equal is acceptable. Wall to ceiling protection <br /> required behind dishwashers. Plastic coated hazdboard is not acceptable. Backsplash height is measured <br /> from the top of the sink basin. <br /> 7. Plumbing must meet state and local codes. <br /> 8. The ventilation system shall be installed and operated to meet applicable building, mechanical, and fire <br /> codes. <br /> A preoperational inspection is required prior to opening for business. At the time of inspection the <br /> construction of the food service establishment must be complete and all equipment must be in place. <br /> Incomplete construction may result in a $60.00 reinspection fee. Please contact the Food Program oRtce at <br /> least one week in advance to schedule an appointment. This will ensure compliance with the Rules and <br /> ReAulations of the State Board of Health for Food Service Sanitation. <br /> If there are any significant changes or additions to your layout or equipment, the Snohomish Health District <br /> must be notified. <br /> Please contact me if you have any questions.
