^A 7 <br />STATEMENT OF WORK <br />Emergency Management Performance Grant (EMPG) <br />Federal Fiscal Year 2005 (FFY05) <br />Exhibit B <br />#E05 - <br />INTRODUCTION: The Washington State Military Department (Military Department), Emergency Management <br />Division (EMD), receives grant monies each year from the Department of Homeland Security (DHS) through the <br />Emergency Management Performance Grant (EMPG). A portion of this grant has been identified by the state to <br />be passed through to local jurisdictions that have emergency management programs to supplement their <br />operating budget. Each jurisdiction that applies and meets the qualifications specified in WAC 118-09 is awarded <br />a grant based on the size of their jurisdictions' emergency management operating budget. Funds are utilized by <br />local jurisdictions to enhance their emergency management capability. <br />The Statement of Work outlines the objectives, goals, and deliverables the Contractor has identified to be <br />performed for FFY05 EMPG. The Timeline outlines the schedule that the Contractor agrees to in order to fulfill <br />the grant deliverables during the contract performance period. The Budget Sheet outlines the Contractor's <br />emergency management operating budget (excluding federal funds) that is dedicated to emergency management <br />operations. In addition, the Budget Sheet is used to announce the FFY05 EMPG award and serves as <br />documentation for the local match to the federal funds. <br />The Contractor Agrees To: <br />1. Complete the objectives, goals, and deliverables as outlined in Attachment A. <br />2. Report progress on deliverables and addressed in Attachment A and advise the Military Department, in <br />writing, of necessary adjustments to the content of the contract. Performance report will be submitted <br />on format provided by the Military Department. <br />3. Submit the Initial Strategy Implementation Plan (ISIP) and Biannual Strategy Implementation Report <br />(BSIR) to the on-line Grant Reporting Tool(https://www.reporting.odp.dhs.gov). <br />4. Make all supporting documentation to reports and billings available to Military Department staff for <br />review, upon request. <br />5. Submit signed, approved invoice vouchers (state form A-19) within 30 days of the end of the month in <br />which the work was performed. Each billing must include a completed Reimbursement Spreadsheet <br />(Attachment B). <br />The Military Department Agrees To: <br />Provide staff coordination and input such as technical information and assistance. <br />2. Review reports and billings for accuracy and completion. <br />3. Reimburse the Contractor within 30 days of receipt and approval of signed, dated invoice voucher(s) <br />(state form A-19), documentation of satisfactory completion of tasks -to -date, and documentation of <br />costs. <br />6. <br />EMPG FFY 2005 Page 9 of 18 City of Everett EM <br />E05-233 <br />
