The Health District operating permit application process must be completed prior to opening for <br /> business. <br /> 2. The dishwash sink is in separnte room. It is unclear wherher or not thc walkway into this room has a <br /> closable door. An additional handa�ash sink is req�ired in the dishwasli room if the entryway has a <br /> closable door. <br /> 3. A reduced pressure backFlow prevention device is required at the end of the copper water pipe serving the <br /> pop dispensing system prior to the carbonation device. <br /> 4. Water heaters must be of sufficient size to provide hot water to dishwasher and/or scullery sinks and at the <br /> same time provide hot water ro all handwash sinks. <br /> 5. All light 6xtures in food preparation and srorage areas must be provided with covers or shatter proof <br /> bulbs. <br /> 6. Plumbing must meet state and local codes. <br /> 7. The ventilation system shall be installed and operated to meet applicable building, mechanical, and fire <br /> codes. <br /> A preoperational inspection is required prior to opening for business. At the time of inspection the <br /> construction of the food service establishment must be complete and all equipment must be in place. <br /> Incomplete construction may result in a $130.00 reinspection fee. Please contact the Food Program office a <br /> minimum of one week in advance to schedule an appointment. This will ensure compliance with the Rules <br /> and Reeulations of the State Board of Health for Food Service Sanitation. <br /> If there are ary significant changes or additions to your layout or equipment, the Snohomish Health Disvict <br /> must be notified. <br /> Please contact me if you have any questions.
