Originally submitted manuFac.urer names and model numbers should be rechrckcd to assure <br />the information submitted is accurate. <br />6. Soups arc indicated as a menu item but the supplier for the soups is not given. The name a�id address of the <br />soup supplier must be submitted. <br />7. 17ie proposed menu indic�tes that fresh unpasteurized juices will be served. When unpasteurized juices are <br />offered for service or [or sale as ready ro ear, rhese foods must be clearly idendfied as unpasteurized cn the <br />menu or on a sign clearly visible to the pac:ons. <br />8. Resubmit a revised tloor plan, drawn to scale, showing locauon of all equipment, plumbing fixmres and the <br />like that includes the requried sinks and infarmation.
