Stars and Stripes Espresso, 9122 Evergreen Way, Evcrett <br />Dear Messers Kardack and Fisher: <br />Your plans have been reviewed �vith the Rules and Reoulations of the State Board of Health, and with <br />the policies of the Snohomish Health District. With the addition of the followin�, the plans are <br />approved. <br />1. Three compar[ment sinks must have integral drainboards at both ends. <br />2. A mop sink or utility sink is required. <br />3. A horizontal separation of at least 16 inches or a vertical partition 16 inches in height is required <br />benveen the food preparation sin!: and all other sinks. <br />-4. Water heaters must be of sufficient size to provide hot water to dishwasher and/or scullery sinks and <br />at the same time provide hot watcr to all handwash sinks. <br />�. Extra wall protection is required on walls behind sinks and food preparation tables. A 16-inch high <br />backsplash of plastic laminate, fiberglass reinforced plastic or equal is acceptable. The floor must be <br />surfaced with a durabte, nonabsorbent, easily cleanable material. <br />6. The ceilin� above the food preparation areas must be nonabsorbent, smooth, and easily cleanahle. <br />7. All lisht fixt ires in food preparation and storage areas must be provided with covers or shatterproof <br />bulbs.� <br />8. Plumbing must meet state and local codes. <br />A preoperational inspection is required prior to opening for business. At the time of inspection the <br />construction of the food service establishment must be complete and all equipment must be in place. <br />Incomplete const-uction may result in a 5130.00 rcinspection fee. Please contact the Food Program <br />ofCce a minimum of one week in advance to schedule an appointment. This will ensure compliance <br />with the Rules and Re�ulations of the State Board of Health for Food Service Sanitation. <br />If there are any significant changes or additions ro your layout or equipment, the Snohomish Health <br />District must be notified. <br />Please contact me if you have any qucstions.
