Proposed, Chanh Duc Restaurant, 9605 Evergreen Way, Everett �_,_, �,� , <br />Dear Mr. Nguyen: <br />Your plans have been reviewed with the Rules and Regulations of the State Board of Health, and with the <br />policies of the Snohomish Health District. With the addition of the following, the plans aze approved. <br />l. An additional handwash sink is required in the food prep and scullery area. <br />2, A horizontal sepazation of at least 16 inches or a vertical partition 16 inches in heieht is required <br />beaveen the food preparation sink and all other sinks. <br />3. This establishment has only one Food prep sink. In addition, only two refrigerators are in use, both <br />used for storage. As a result the ability of this establishment to cool foods is limited. Because of this <br />one of the following is required: <br />a. Install an additional iood preparation sink <br />b. Reduce menu by at least fifry percent. <br />ii menu is reduced. a copy of the new menu must be submitted to the Health District prior to <br />approval of the remodel. <br />Eli�ninate advancec cooking and cooling of foods. Cook only enoueh for each days use. <br />d. Install a walk-in refrigeraror. <br />A preoperational in�pectio:� is required prior to opening for business. At the time of inspection the <br />construction of the food s�rvice establishment must be complete and all equipment must be in place. <br />Incomplete construction may result in a$60.00 reinspection fee. Please contact the Food Progra:n <br />of�ce at least one week in advance to schedule an appointment. This will er.sure compliance <br />with the Rules and Re�ulations of the State Board of Health for Food Service Sanitation. <br />If there �ue uny signiticant changes or additions to your layout or equipment, the Snohomish Health <br />Disvict must be notified. <br />Plea�e contact me if ycu have any questions.
