OF WORK: <br />All test holes will be executed using our AirNacuum excavation system. CHS <br />Engineers, LLC has chosen approximately 17 test hole locations on various water lines for the <br />above project. <br />APS, Inc. will not bc held liuble for final restoration (i.e. overlay/ne�o sideH�alk panel, <br />e�c.) As this cost has not been accounted for in this estimate. Each test hole opening under <br />asphalt or concrete will be temporarily repaired with either concrete or polymer adhesive asphalt <br />patch. Each test holc will carry a 2-year wartanty against failure f�om the date of execution. <br />APS, Inc. shall r.rovid�� the standard o�care to repair each test hole as close as possible to the <br />existing surlacc condition. <br />All information gathered on utilities will includc top, bottom, width, general soil <br />conditions and asphalUconcrete thickness. <br />Some utility infonnatiou tnay not be obtainable duc to depUi, and or conductibilify of <br />pipes. Information collectcd on utilities will be retumed to Eli Zehner, P.E. with CHS <br />Engineers, LLC in an excel forniat. <br />At the completion of each test hole APS, Inc. will place a utility marker at grade showing <br />thc location and dcpth of utilities, or structures found. <br />1-3 <br />
