Tasks iocluded in t6e Utilitv Potholioe service <br />Y Initiate "One-Call" request & interface with locators and Utility companies. <br />i= Submit traffic control plans to the City, State or County as required. <br />➢ Obtain Right of Way use permits as required. <br />D Open test hole in asphait or concrete no lazger then S"x 8", by methad of jackhammer or <br />coring. <br />Y Expose utilities at each test hole location. <br />Y Obtain top, bottom and width of utility or structure measurements as required. <br />➢ Retum na4iroe-sve�eE' ��l to the t st hol �req iu red. <br />➢ Restore test hole in appropriate lifts using pneumatic compactor. <br />➢ Place a polymer asphalt patch or concrete over each test hole as required. <br />Y Place washer or hub at each test hole location designating test hole number and peRinent <br />utility information. <br />1= Providc utility infortnation on an Excel spreadsheet and / or test hole data sheet. <br />APS Proiect# 1044 VAC <br />
