I was advised by
Officers on scene that the driver had been extracted from lhe veh';le by Everett FD units. The driver was
being treated in the back of an EFD medic unit.
I contacted Officer Klocker who had begun investigatfng the collision. Officer Klocker informed me that a
witness to the collision, Toren Hoyle, saw unit 01 traveling EIB on Edward Ave toward Friday Ave at
approximately 30 MPH. Hoyle said that unit 01 did not stop at the stop sign at Friday Ave,went throuyii
the intersection, over the curb, and into the yard. Unit 01 struck a fire hydrant as it entered the yard.
Officer Klockerollected digital Images documenting the collision scene. Hoyle completed a wriUen
statement. The vehicle was towed from the scene by American Towing.
I followed the medic unit to the hospital where I contacted the driver of unit 01. He identified himself as
Steven H. Oyen dob: 9111/51. Oyen had multiple facial lacerations from the collision. I asked Oyen what
had caused the collision. Oyen said that he did not know. He stated that he was slowly going EIB on
Edward Ave toward Friday Ave. Oyen told me that he did not remember the collision at all and only
remember traveling E/B on Edward Ave. I did not smell any odor of intoxicants on Oyen's breath as he
spoke. His voice was ciear and he answered questions comp!tely and appropriateiy. I saw that his
puplls were the same size and did not appear dilated or constricted.
I provided Oyen with the case number for the collision and the tow company that has his vehicle. I
instructed Oyan to call 911 when he is capable and provide an OKcer with his (nsurance company and
policy number to be added to this report.
I CEflT7FY(CECI.ViE)NCER PENALTY CF PERJURY UYOER TME LA1VS OP TME 9taTE CF 1YASNIN6TON iNAT TNE FORF60N019 7RUE AND CC1iRECL(RCw 9A.T2.OB5)
B.YEADDN 17•1Q•1I Of:l!AM
IhVESTIGAnNGOFFICER'S.iGW1NnE unITORDSLCET OAiEo Pld:ESiGEO
MPP'JYTYY
ymema:ir.eM
N.CAFMAN I11!
BNJGE dP iO 1711 Onl WAOD10700 iKP0uLE051'MO@0 l:71 PM iiME POIitERRWEO!:1]PM
PART B uaawim n pap PAE OF I
