CHECKLIST REVIEW #115 PROPOSAL To renovate 9,250 square foot two story brick building at 1918 Everett Ave. RECOMMENDATIONS It is the recommendation of the Public Works Department that issuance of a Negative Declaration be subject to the following: 1. Access, circulation and parking plans are to be submitted to the Public Works Department for review and approval. Plans shall include all abutting public right of way and show all public improvements required. 2. City streets are to be kept clear of dirt and debris during construction. 3. During construction al: truck traffic to and from the site �vithin the City of Everett shall be on arterial streets unless othenvise approved by the Public Works Department. 4. Wheelchair ramps on both sides of Everett on Lombard and on both sides of Lombard at Everett are required. New driveway access at alley approach on Lombard is required. Cracked and sunken sidewalk on Lombard shall be replaced. \Vorn spot in alley shall be repaired. S. A drainage plan sho�ving any change in the area of impervious surface is required. � 6.
