With the addition of the follcwine, the plans are approved. <br /> 1. The Health District operating pernut application process must be completed prior to opening for <br /> business. <br /> 3. A copy of the menu must be submitted. <br /> 3. An additional handwash sink is required in the prep sinWscullery area. <br /> 4. A reduced pressure backflow prevention device is required at the end of the copper water pipe serving <br /> the pop dispensin� system prior to the carbonation device. <br /> >. A horizontal separation of nt least I6 inches or a vertical p<u-tition 16 inch�s in height is required <br /> between the food preparation sink and all other sinks. <br /> 6. Water heaters must be of sufficient size to prcvide hot water to dishwasher and/or sculle.y sinks and at <br /> the same time provide hot water to all handwash sinks. <br /> 7. All food service equipment must be listed by the National Sanitation Foundation (NSF) for its intended <br /> use. The soup warmer and crock pot(Star Modef #7C-B7 for eacl:) .ue not found in the current NSF <br /> listin�s. These units must bc replaced with NSF listed equipmen!. <br /> S. The ceiling above the food preparalion areas must be nonabsorbent,smooth, �nd easily cleanable. <br /> 9. All light fixtures in food preparation and srorage areas must be provided with covers or shatter proof <br /> bulbs. <br /> 10. Plumbing must meet state and local codes. <br /> l 1. The ventilation system shall be installed and operated to meet applicable buildin�, mechanical, and fire <br /> codes. <br /> A prcoperation:il inspection is required prior to opening for business. At the ume of inspection the <br /> construction of the food service establishment must be complete and all equipment must be in place. <br /> Incomplete construction may result in a S60.00 reinspection fee. Please contact the Food Program <br /> office at least one week in advance to schedule an appointment.
