Everett <br />Dear Mr. Tsan: <br />Your plans have been reviewed with the Rules and Regulations of the State Board of Health, and with the <br />policies of the Snohomish Health District. With the addition of the following, the plans are approved. <br />I. The Health District operating permit application process must be completed prior to opening for <br />business. <br />2. An indirect waste is required for the food preparation sink, mechanical dishwasher, ice machine, <br />running water dipper well, and equipment in which food is placed. <br />3. A horizontal separation of at least 16 inches or a vertical partition 16 inches in height is required between <br />the food preparation sink and all other sinks. <br />4_ Water heaters must be of sufficient size to provide hot water to dishwasher and/or scullery sinks and at <br />the same time provide hot water to all handwash sinks. <br />5. The dishwasher must be large enough to hold the largest item needing washing. <br />6. Extra wall protection is required on walls behind sinks and food preparation tables. A 16 inch high <br />backsplash of plastic laminate, fiberglass reinforced plastic or equal is acceptable. Floor to ceiling <br />protection is required on wall behind dishwashers. Plastic coated hardboard is not acceptable. <br />7. The floor must be surfaced with a durable, nonabsorbent, easily cleanable material. <br />8. All light fixtures in food preparation and storage areas must be provided with covers or shatter proof <br />bulbs. <br />9. Plumbing must meet state and local codes. <br />10. The ventilation system shall be installed and operated to meet applicable building, mechanical, and fire <br />codes. <br />11. The Tapan Grill areas lack facilities for long term food storage. Only food for immediate service or <br />cooking is permitted at the grill areas <br />A preoperational inspection is required prior to opening for business. At the time of inspection the <br />construction of the food service establishment must be complete and all equipment must be in place. <br />Incomplete construction may result in a S62.00 reinspection fee. Please contact the Food Program <br />office a minimum of one week in advance to schedule an appointment.
