Wimi the addition of the <br />following items, the plans are approved. <br />1 . A properly vented dual check valve device or an approved re:l ned pressure backflow <br />assembly between copper pipe or tubing and carbonated beverage dispensing machines is <br />required. <br />2. ''he Health District operating permit application process must be completed prior to <br />opening for business. An application ar.d fee schedule are enclosed far your use. <br />If there are any significant changes or a_0c itions to your layout or equipment, the Snohomish <br />Health District must be notified. <br />A pre -operational inspection is required prior to opening for business. Please contact the Food <br />Program office about one week in advance to schedule an appointment.
