For Chicken Lovers, 1402 Everett Mall Way #1FA, Everett <br />Dear Mr. Beszhak: <br />Your plans have been revim-od for compliance with the Rules and Regulations of the State Board <br />of Hoalth and with the policies of the Snohcmish Health District. Wimi the addition of the <br />following items, the plans are approved. <br />1 . A properly vented dual check valve device or an approved re:l ned pressure backflow <br />assembly between copper pipe or tubing and carbonated beverage dispensing machines is <br />required. <br />2. ''he Health District operating permit application process must be completed prior to <br />opening for business. An application ar.d fee schedule are enclosed far your use. <br />If there are any significant changes or a_0c itions to your layout or equipment, the Snohomish <br />Health District must be notified. <br />A pre -operational inspection is required prior to opening for business. Please contact the Food <br />Program office about one week in advance to schedule an appointment. 'This will insure <br />compliance with the Rules and Regulations of the State Board of Health for Food Service <br />Sanitation (WAC 246-215). <br />Please contact me if you have any questions.
