Dispensing freezers <br />require wash kits (hot and cold water) on or adjacent to the machine, <br />10. Adequate facilities must be provided fcr the orderly storage of employee's <br />clothing and personal belongings. <br />11. The bakery, deli, and candy area will require a review by the health <br />department, unless foods are limited only to packaged items. <br />12. The plan review was made from information you presented to our office. <br />Therefore, should there be any major changes and/or additions to your <br />layout plans and equipment specifications, you must notify the Snohomish <br />Health District of the changes. <br />13. It should be pointed out that the health department approval of the finished <br />installation (equipment and facilities) is subject to the final field <br />inspection. You should call, wher ready, for the final field inspection. <br />In the meantime, the health department will make field checks of the <br />construction. <br />14. In 1980, the Snohomish Health District's Board of Health established a plan <br />review charge for review of plans and specifications. The fee is $25.00 per <br />hour. Your total review time is eight (8) hours or $200.00. The fee must <br />be paid before the food -service establishment permit is issued. <br />The Snohomish Health District world suggest that an office appointment be made <br />with you and/or the food -service designer so we could discuss your plans and <br />the above items. <br />Very truly yours, <br />John W.
