With the addition of the following, the plans are approved. <br />1. A reduced pressure bac!cflow prevention device is required at the end of the copper water pipe serving <br />the pop dispensing system prior to the carbonation device. <br />3. All �iah: fixmres in food preparation and stora�e areas must be provided with covers or shatter proof <br />bulbs. <br />A preoperational inspection is required prior [o opening for business. At the time of inspection the <br />construction of Ihe food service establishment must be complete and all equipmeut must be in place. <br />Incomplete constn�ction may result in a 560.00 :einspection fee. Please contact the Food Prugram <br />oFCce at least one weelc in advance to schedule an appointment. This will ensure comoliance <br />with the Rules and Re�ulations of the State Board of Health for Food Service Sanitation. <br />If there are any si�nificant chanoes or additious to your layout or equipment, the Snohomish Health <br />District must be notified. <br />Please contact me if you have any questions.
