Your plans have not been approved due to the toliowing deficiencies. <br />When these items have been corrected, the Disirict will reconsider your application for <br />plan approval. <br />1. Piease provide a complete finish scheduie of walls and ceiling. <br />2. A tood preparation sink is required if iresh vegetabies are prepared as a standard <br />menu item. Sinks used tor handwashing or equipment or uten;il washing shall not be <br />used tor food preparation. There shall be no direct connections between ihe <br />sewerage syslem and any drains originating irom equipment in which food is placed. <br />This includes ice machines and vegetable prep sinks. <br />3. A mop sink is needed tor this establienment. Submit proposed location. <br />4.
