With the addition of the following, the plans are approved. <br />1. The Health District operating permit application process must be completed prior to opening for <br />business. <br />2. An indirect waste is required for the food preparation sink, mechanical dishwasher, ice machine, <br />running water dipper well, and equipment in which food is placed. <br />3. A reduced pressure backflow prevention device is required at the end of the copper water pipe serving <br />the pop dispensing system prior to the carbonation device. <br />4. A horizontal separation of at least 16 inches or a vertical partition 16 inches in height is required <br />between the food preparation sink and all other sinks. <br />5. Water heaters must be of sufficient size to provide hot water to dishwasher and/or scullery sinks and at <br />the same time provide hot water to all handwash sinks. <br />6. All food service equipment must be listed by the National Sanitation Foundation (NSF) for its intended <br />use. The Thunder rice cooker, model number GSC R003, (item #1 I on the equipment list) is not found <br />in the current National Sanitation Foundation ( NSF) or equivalent listings. This unit must be replaced <br />with an NSF or equivalent listed unit or documentation must be submitted which demonstrates NSF <br />equivalency. Manufacturers name and model number for a replacement unit must be submitted. <br />7. Extra wall protection is required on walls behind sinks and food preparation tables. A 16 inch high <br />backsplash of plastic laminate, fiberglass reinforced plastic or equal is acceptable. Floor to ceiling <br />protection is required on wall behind dishwashers. Plastic coated hardboard is not acceptable. <br />8. All light fixtures in food preparation and storage areas must be provided with covers or shatter proof <br />bulbs. <br />9. Plumbing must meet state and local codes. <br />W. The ventilation system shall be installed and operated to meet applicable building, mechanical, and fire <br />codes. <br />A preoperational inspection is required prior to opening for business. At the time of inspection the <br />construction of the food service establishment must be complete and all equipment must be in place. <br />Incomplete construction may result in a $130.00 reinspection fee. Please contact the Food Program <br />office a minimum of one week in advance to schedule an appointment. This will ensure compliance <br />with the Rules and Reeulations of the State Board of Health for Food Service Sanirnrinn_ <br />
