The Health District operating permit application nrocess must be completed prior to upening for <br />business. <br />2. Prior to permit issuance a copy of the menu must ue submitted. <br />3. A horizontal separation of at least l6 inches or a vertical partition l6 inches in height is required between <br />the food preparation sink and all other sinks. <br />�3. Wuter heaters must be of sufficient size to provide hot water to dishwasher and/or scullery sinks and at the <br />same time provide hot water ro all handwash sinks. <br />�. All food service equipment must be listed by the National Sanitation Foundation (NSF) for its intende� <br />use. Note: The deli display case (Item #5 on sheet Al, TRUE #TS10-d8-4-L) is listed for the storage of <br />prepackaged food only. <br />6. Extrn wall protection is required on walls behind sinks and food preparation tables. A 16 inch high <br />backsplash of plastic laminaze, fibergl:us reinforced plastic or equal is acceptable. Floor to ceiling <br />pro[ection is required on wall behind dishwashers. Plastic coated hardboard is not acceptable. <br />7. All light fixmres in food preparation and storage areas must be provided wi�h covers or shatter proof <br />bulbs. <br />3. Plumbing must meet state and local codes. <br />9. The ventilation system shall be installed and operated to meet applicable building, mechanical, and fire <br />codes. <br />A preoperational inspection is required prior to opening for business. At the time of inspection Ihe <br />construction of the food service establishment must be complete and all equipment must be in place. <br />[ncomplete construction may result in a 562.00 reinspection fee. Please contact the Food Program o�ce a <br />minimum oF one week in advance to schedule an appointment. This will ensure compliance with the Rules <br />and Re2ula[ions of the State Board of Health for Food Service Sanita[ion. <br />If there are any signi�cant changes or additions ro your layout or equipment, the Snohomish Health District <br />must be notified. <br />Please contact me if you have any questions.
