Rervir,es <br />Subject: Change of Ownership — A A China Buffet (formerly Galaxy Casino), 607 SE Everett Mall Way #I, Gvemtt <br />Dear Sir or Madam: <br />The Snohomish Health DisMct has received your application for food service establishment operating permit lbr <br />rcopening of A A China Ruffet (formerly Gslaxy Casino) restaurant at 607 SE �verett Mall Way #1 in Lverett. Prior to <br />operating permit issuance and approval to open for business, the following is required. <br />]Iealth District mcords indicate that this facility has been closed since December 18, Z005. Also, you are proposing a <br />change to a Chinese style menu. Furthermore, the previous establishment, Galaxy Casino, had twenty-eight seats. You arc <br />proposing to increase this to one hundred and sixty-eight seats. Because of these items, a preoperational inspection of the <br />laciliry must be completed prior to pemiit issuance and approval to open for business. The preoperational inspection will <br />detern�ine what facility changes and or upgrades are required prior to opening for business due to the menu change and <br />proposed increase in seating. Please note that part of the requirements for reopening �vill include additional refrigeration <br />due to the increased volume of food associated with the proposed seating increase. An application for inspection to <br />reopen a clased food service establishment is enclosed. <br />Prior to the request £or the preoperatioual inspection, a description of the food preparation process (HACCP) for the thrce <br />anticipated mast popular food items From your menu must be submitted. Lxamples for tl�e HACCP are enclosed. <br />If any remodel of this facility is proposed, plans and specifications must be submitted to the Health District for review <br />prior to start of construction. If a remodel is currently undenvay, thcn construction must cease until plans have been <br />reviewed and approved by the Health District. All new equipment, including buffet tables, must be approved by the <br />Health District prior to installation. A plan review inforniation packet is enclosed. <br />Pinally, the permit fee submitted is incorrect. The fee submitted was for a transfer of permit. No permit has been issued <br />for this facility in 3006.
