The Health District operating permit application process must be completed prior to opening for <br />business. <br />3. Submit copy af ineni. <br />3. Water heaters mutit be of sufficient size to provide hot water to dishwasher and/or scullery sinks and at <br />the same time provide hot water to all handw:ish sinks. <br />4. This information must be <br />submitted prior to the preoperational inspection. <br />The proposed True glass door merchandiser, model #GDM47, is NSF listed for the storage of <br />prepackaged and bottled products only. <br />Extra wall protection is required on walis behind sinks and food preparadon tables. A 16 inch high <br />backsplash of plastic laminate, fiberglass reinforced plastic or equal is acceptable. <br />The floor must be surfaczd with a durable, nonabsorbent, easily cleanable materi:il. <br />10. C:upet is shown as the proposed floor cover in the retail sales area. Carpet is not an easily cleanable <br />material. The ca�pet must be replaced with a durable, nonabsorbent, easily cleanable material. <br />I 1. The ceiling above the food preparadon areas must be nonabsorbent, smooth, and easily cleanable. <br />t'_'. All light fixtures in food preparation and storage areas must be provided with covers or shatter proof <br />bulbs. <br />13. In the retail sales area it is unclear whether or not the food items :ue handled by employees or if this <br />will be customer self serve. If foods are to be handled by employees, a handwash sink must be installed <br />at the front counter. If this is to be a customer self service area, then proper precautions must be <br />implemented so as to prevent direct hand contact of the food by the customer. <br />14. Plumbing must meet state a�id local codes. <br />15.
