With the addidon of the following, the plans are approved. <br />1. The Health District operating peanit applica6on process must be completed prior to opening for <br />business. <br />2. All food service equipment must be listed by the National Sanitation Foundation (NSF) for iu intended <br />use. <br />3. Storage refrigerators must be ]isted for the storage of potentially hazazdous foods. <br />4. Extra wall protection is required on walls behind sinks az�d food prepazation tables. A 16 inch high <br />backsplash of plastic larrunate, fiberglass reinfor�ed plastic or equal is acceptable. Plasdc coated <br />hardboard is not acceptable. <br />5. Plumbing must meet state and local codes. <br />6. The ventilation system shall be installed and operated to meet applicable building, mechanical, and fue <br />codes. <br />A preoperational inspection is required prior to opening for business. At the time of inspection the <br />construction of the food service establishment must be wmplete and all equipment must be in place. <br />Incomplete construction may result in a$60.00 reinspection fee. Please contact the Food Program <br />ofCce at least one week in advance to schedule an appointment. This will ensure compliance <br />with the Rules and Regulations of the State Board of Health for Food Service Sanitation. <br />If there are any significant changes or additions to your layout or equipment, the Snohomish Health <br />District must be notified. <br />Please contact me if you have any questions.
