EXHIBIT A <br /> SCOPE OF WORK 4 <br /> 1. Work to Be Performed <br /> Each City of Everett CHIP housing rehabilitation project, over $5,000 in value, built <br /> prior to 1978, when Lead Based Paint was commonly used, must be tested for the <br /> presence and condition of Lead Based Paint on every interior and exterior surface. <br /> The contractor will perform Lead Based Paint XRF inspections, risk assessments, on- <br /> site management, air monitoring and clearance inspections. <br /> Work includes: <br /> • Mike Hara, an EPA Certified Inspector/Risk Assessor performing all work <br /> (253) 566-9377 <br /> • Site testing with the MAP4 Spectrum Analyzer and one copy of Lead Based <br /> Paint Survey test results <br /> • If Lead Based Paint is present, a definitive scope of work and options are <br /> provided for CHIP Housing Inspector/Advisor and the contractor's use <br /> • Inspection of the project during the course of the work and at the end, <br /> performing all required clearance sampling and providing one copy of the <br /> report to the owner <br /> • Performing wipe and soil sampling, including delivery costs and lab analysis <br /> • Prepare scope of work for Lead Based Paint bidding purposes <br /> • Solicit contractors for bidding and conduct project walk-through <br /> • Receive contractor bids and prepare analysis for Owner of project <br /> • On site observation of contractor to ensure compliance of regulations <br /> • Teach "HUD Safe Work Practice Class" for contractors: 8 hours (4 hrs <br /> instruction, 4 hrs. hands-on training( <br /> • On site air monitoring, if applicable <br /> 2. Reports <br /> Report Frequency Contents <br /> Lead Based Paint Survey for Each eligible project Executive Summary, Proposed Scope of <br /> CHIP (approx. 30 per year) Work, Report Raw Data, Preliminary <br /> XRF Results, XRF and Lab Results, <br /> Summary Analysis, Confirmed Positive <br /> Results, Calibration Log, Performance <br /> Characteristics Sheet, Certifications <br /> 45 <br />
