CITY <br /> CITY OF EVERETT <br /> PROFESSIONAL SERVICES AGREEMENT <br /> Effective Date: December 15,2016 <br /> Consultant: Name Northwest Fire Training Group <br /> Address 1001 Curtis Street,Sedro Woolley,WA 98284 <br /> Phone (360)661-1508 <br /> Fax (855)282-0730 <br /> Email firechairl@me.com <br /> UBI No. 602756571 <br /> Everett Business <br /> License No. 056330 <br /> Scope of Services: Promotional Examinations. <br /> The Northwest Fire Training Group(the Consultant)will provide trained evaluators to conduct the practical <br /> skills examinations in a competent and professional manner for the classification of Firefighter/Driver. The <br /> Consultant's evaluators will assess the critical skills for the Everett Fire Department's driving and pumping <br /> apparatus, based on criteria developed by the Consultant and the City's Human Resources and Fire <br /> Departments. <br /> In the event of challenges to the examination,the Consultant will support and defend its proprietary testing <br /> materials and practices,and will assist the City in addressing any challenges. <br /> Without an amendment to this Agreement,the Consultant shall not perform any services that are in addition <br /> to, or beyond the scope of,this Agreement. <br /> Time of Beginning and Completion of Performance. This Agreement shall commence as of the date of <br /> execution of this Agreement and shall be completed by June 30,2017. <br /> Compensation. The City shall pay the Consultant only for services actually rendered. Such payment shall <br /> be full compensation for the services rendered, including, but not limited to, all labor, materials, supplies, <br /> equipment and incidentals. The Consultant shall be paid as follows: (1)Point of Contact/Senior Evaluator: <br /> $250.00 per day; (2)Evaluator: $200.00 per day each,up to five(5)evaluators on site per day. <br /> The Consultant may receive payment as reimbursement for Eligible Expenses actually incurred. "Eligible <br /> Expenses"means those types and amounts of expenses approved for reimbursement by the City.If approval <br /> for reimbursement is not obtained from the City prior to the Consultant's incurring the expense, the <br /> Consultant acknowledges that the City retains the option not to reimburse the Consultant. If, and to the <br /> extent, overnight lodging in western Washington is authorized, the Consultant is strongly encouraged to <br /> lodge within the corporate limits of City. When authorized, the Consultant will be reimbursed 100% of <br /> lodging expense, if lodged within the corporate limits of the City, but the Consultant will be reimbursed <br /> 50% of lodging expense when lodged outside the corporate limits of the City. If authorized, the City may <br /> (at its sole option)obtain or arrange air travel for the Consultant. Eligible expenses shall not exceed$0.00. <br /> Page 1 <br />
