Carpet quality "Qyers Hall" by Shaw is approved <br /> for a direct glue down installation. <br /> His review is pursuant to WAC 246-�16 Boarding Homes Codified Rules, Regvlations and Standards <br /> adopted by the department as amended to date. <br /> I have enclosed two notification cards. Please complete and return the green card when installation Uegias <br /> and the pink card when installation is complete. Staff will schedule all necessary State Fize Marshal and <br /> Department of Health inspections upon receipt of the pink card.
